It is NOT the job of the commander to ask for confirmation. Confirmation should be given automatically by the receiver, and that is the issue since more than once the commander actually HAS to ask for confirmation.

I disagree. Especially in the heat of action, it is mandatory and of utmost importance that the commander's orders are implemented immediately, since they usually have better oversight of the situation. Even if you are doing something else, confirm the order and try to implement it as quickly as possible.
